The College of Physicians Malaysia (CoPM) extends its highest appreciation to Prime Minister Anwar Ibrahim, Health Minister Dzulkefly Ahmad, and Chief Secretary to the Government (KSN) Shamsul Azri Abu Bakar, following the historic Budget 2027 announcements.
The policy milestone announced on October 9 to end the contract system starting in 2027 marks the end of a 10-year era of workforce instability introduced in October 2016.
Offering permanent positions to over 9,000 contract doctors upon completing graduate training resolves long-standing issues surrounding job security, promotion pathways, and service continuity for young medical professionals.
Critical Impact On Physicians And Specialist Care
Direct retention boost: Permanent tenure serves as an immediate anchor to retain skilled medical officers within the public sector, halting the critical brain drain of junior talent.
Strengthening postgraduate specialist pipelines: Specialist physician training—via both the Parallel Pathway and Master’s programmes—demands structured, uninterrupted years of clinical experience. Eliminating contract uncertainty provides medical officers with the career stability required to pursue postgraduate specialisation smoothly.
One-off special promotion for 12,000 officers: CoPM strongly advocates that a significant portion of the newly announced promotions directly addresses the plight of long-serving medical officers and specialists.
The one-off special promotion exercise for civil service officers—extending up to Premier Grade (Jusa) positions—must resolve long-standing promotion bottlenecks for these doctors who have been awaiting promotion for a long period and have an outstanding performance.
Recognising their dedicated service boosts workplace morale and ensures our public healthcare institutions retain experienced clinical leaders.
Improved living conditions via an RM2.3 billion quarters allocation: The RM2.3 billion commitment to build and maintain civil servant quarters significantly benefits physicians stationed in rural, district, or high-cost areas.
Providing safe, comfortable housing directly alleviates financial strain, enhances work-life stability, and encourages physician doctors to accept relocations across the country.
Meeting future health care demands: Expanding the pool of physician trainees and securing senior talent is vital to bolstering health care resilience, managing non-communicable diseases (NCDs), and addressing the needs of an aging population.
Empowering The Multidisciplinary Clinical Team
Increased paramedic and nursing post-basic incentive (RM200/month): Raising the post-basic allowance for over 47,000 nurses and paramedics directly strengthens the core clinical team supporting physicians in wards, intensive care units (ICUs), and specialised clinics. Highly trained clinical support staff are indispensable for physicians to deliver high-quality specialised care.
400 new ambulances: Upgrading the emergency response fleet ensures faster pre-hospital stabilisation and swift transport of critically ill patients to tertiary medical units managed by physician specialists.
A Modernised Health System
CoPM also strongly endorses the total RM47.7 billion health care allocation, particularly the RM1 billion investment to roll out Electronic Medical Records (EMR) nationwide, RM1.2 billion for facility maintenance, RM770 million for modern equipment (including haemodialysis), and enhanced allowances for frontliners.
While CoPM sincerely lauds and deeply appreciates these significant commitments made in the face of challenging economic times, we must recognise that continuous, progressive investment will remain essential.
As our demographic shifts and clinical complexities grow, more resources and sustained policy support will still be needed over time to ensure that all Malaysians continue to receive the world-class care they deserve.
By shifting from piecemeal measures to a decisive structural solution, the government has laid a secure foundation for the future of Malaysian medicine and must continue to deliver upon its promises.
CoPM remains fully committed to collaborating with the Ministry of Health to expand specialist training, drive health care reforms, and elevate clinical excellence nationwide.
