Bandar Kuching MP Dr Kelvin Yii says BIW isn't a mere “allowance”, but symbolises regional equity and appreciation for health workers' sacrifice. He says the annual RM4m needed to restore the progressive allowance is worth the investment to retain doctors.

Bandar Kuching MP Dr Kelvin Yii urges MOH and JPA to reconsider BIW allowance cuts for doctors, pharmacists, and nurses posted to Sabah, Sarawak, and Labuan. Ideally, the old BIW framework (percentage of salary) should be restored for health care workers.
Under SSPA, the BIW allowance is cut to a fixed RM360 monthly rate for new P&P officers appointed Dec 2024 for postings to Sabah/Sarawak/Labuan, compared to the old BIW rate of percentage of salary. UD14 medical officers face 64% disparity in BIW payments.
According to MOH data, only 432, or 57%, of 764 contract medical officers reported for duty for permanent placements in Sarawak this year as of November, indicating that 332 doctors were no-shows or rejected permanent appointment offers.
Dr Dzul says medical officers can claim new ETAP rates on Nov 1. MOF is considering transfer allowances for contract MOs offered permanent posts in Sabah and Sarawak. Malaysia is also finalising bilateral deals on ethical recruitment of health personnel.
